How I feel for you. My dog has kidney problems and I've been through the mill with him. Not quite as bad as you tho. My first thought is, could you ask your vet if there is any medication to control his urine output? I know there is something available for incontinent bitches so it may be worth asking about. Also, if he really can't help peeing,etc in the house, would it be possible to paper or litter train him again, like he did as a pup? He sounds like a really polite, well-mannered chap like my old boy so I wonder if he feels as bad about it as you do. If he does, you could probably show him an area to go and praise him when he goes there so he doesn't feel so bad about it, and you won't have as big a mess to clean up. Probably easier for him to reach than going outside. I have seen puppy training pads too, which are supposed to attract puppies to toilet on them. Maybe you could try that. If he is really badly incontinent and can't hold on that long, you could look to see if there are incontinence pants for male dogs. Again, I've seen them for bitches.

How is his general demeanor? Do you think he is happy or at least contented, or is he suffereing? I think it's a very personal decision only you can make, but your vet should be able to answer alot of your questions about his quality of life, and what he actually physically feels. You say he growls because he's in pain, perhaps some sort of pain relief is needed now. I wish you all the best with him.
